With the
changes
of
health
care
,
due
to the
Affordable
Care
Act
,
insurance
companies
will
no
longer
be
reimbursing
for
hospital
acquired
infections
(HAIs)
Surgical
site
infections
(SSIs)
are
third
in
amount
of
HAI
, and they are the
most
preventable
. The
complications
that
occur
can
lead
to an
increased
cost
in
health
care
. These
increased
costs
are from
treatment
of
SSIs
that
may
cause
increased
lengths
of
stay
,
antibiotic
treatment
and
potentially
the
need
for
additional
surgeries
. This
is
causing
a
loss
in
revenue
for
health
care
facilities
nationwide
,
causing
many
facilities
to
now
evaluate
their
pre-operative
practices
looking
for
new
ways
to
decrease
the
risk
of
patients
acquiring
an
SSI
.
Before
the
age
of
outpatient
surgery
patient
were
given
pre-operative
washes
with an
antiseptic
soap
, these
included
chlorhexidine
,
beta
dine
and
others
. This
practice
was
based
on the
fact
these
washes
decreased
the
amount
of
both
transient
and
resident
bacteria
,
which
are the
major
cause
of
SSIs
. This
project
will
look
at
surgical
pre-operative
wash
with
chlorhexidine
(CHG)
and the
effects
it
has on
decreasing
the
rates
of
surgical
site
infections
. The
implementation
of the
surgical
wash
will
include
presentation
to
key
stakeholders
,
supporting
the
use
of the
pre-operative
shower
with
CHG
performed
the
evening
before
and the
morning
of
surgery
.
Task
force
development
will
help
facilitate
this
practice
. A
business
plan
will be
developed
along
with
presentations
for
upper
management
promoting
the
benefits
of the
surgical
wash
with
CHG
.
Surveys
will be
used
to
assess
staffs
knowledge
about
the
surgical
pre-operative
wash
and the
proper
procedure
on how to
perform
the
wash
. A
presentation
for the
CHG
representative
will also be on
hand
for
staff
education
.
Preparing
the
ACU
staff
to
educate
patients
on the
proper
procedure
,
amount
of
time
the
CHG
should be
left
on
without
rinsing
, to
gain
the
maximum
benefit
for
patients
.
Chart
audits
will be
performed
after
the
implementation
of the
surgical
pre-operative
wash
to
monitor
for
changes
in the
rates
of
SSI
.
Staff
surveys
will be
performed
weekly
to
assess
the
implementation
plan
and
barriers
that
may
interfere
with the
process
.
Patient
surveys
will be
conducted
to
assess
education
methods
,
evaluating
any
areas
that
need
improvement
. The
implementation
of the
surgical
pre-operative
wash
will be
assessed
at
all
stages
looking
for
areas
of
improvement
ensuring
for the
success
of the
implementation
.
